Our facility is seeking an ACT team PMHNP to supply care in the surrounding areas of Brooklyn, NY.
Setting: ACT team
Schedule: Monday - Friday 9am-5pm
- Required on site daily morning meetings (9:30-10:30) and then everyone goes out in the community to see patients after that
Patient Population: Adults
Hours Needed: Full time, 35-40 hours preferred
Assignment Type: Permanent (potentially open to contract)
Additional Requirements:
- Main requirement is that the provider needs to already be enrolled in NYS Medicaid, Medicare and enrolled with PECOS
- They don't have a psychiatrist who can provide supervision, but they are open to NPs who still need this and bring their own collaborating physician

- MAX caseload is 68 (that you would see over the entire month)
- Hard to say how many you would see a day out in the community depending how close they are located to each other. Some days you could do 3-4 that are near each other, other days just 1-2, etc.
